STEP 1 (COMPLETE FORMS)

• Print and complete all 4 pages of these forms  (2 pages CLA Rego & Medical forms and 2 pages ANSW Registration form)

• Check that you have:                                                                                                                                                                                                                                                                                                  

• signed and dated page one of the Athletics NSW registration form;

• signed page two (2) if over 18yrs; and  

• signed page one of the Club registration form. 

        §  Please refer to the registration categories/age groups and payments schedule.

NB: Your age group depends on your age as at the 30/09/2010. If under 18, a parent or guardian MUST also sign and date.

 STEP 2 (PAYMENT)

• Payment can be made by cash or cheque (Cheques must be made payable to "Cherrybrook Athletics") NB: Please do not post cash.

NOTE - IF YOU NEED TO SEND IN YOUR FORMS:

• Or hand deliver to Nicky Jenneke, Valmé Kruger or Catherine Pearce at Greenway Oval on Monday or Wednesday afternoons.
• Or contact Mrs “P” if you wish to deliver them to her directly.

Please note:

• Registration is an involved process, so please allow approximately ten (10) working days for it to be processed.

• Registration entitles you to compete at both club level with Cherrybrook Athletics, and at all events run by both Athletics NSW and Athletics Australia.

• To compete at Masters (30+) State and National titles you must also be a financial member of the NSW Masters association ($40 per annum).
